Solution

During a 1st trimester OB sonogram, the following can be demonstrated and/or performed:

  1. Presence of the secondary yolk sac: The secondary yolk sac is one of the first structures that can be seen in the gestational sac during early pregnancy. It can be identified as early as 5 weeks gestation.

  2. CRL for estimation of gestational age: Crown-rump length (CRL) is the measurement of the length of human embryos and fetuses from the top of the head (crown) to the bottom of the buttocks (rump). It is used to estimate gestational age.

  3. M-mode of heart rate: M-mode ultrasound can be used to measure the fetal heart rate. This is typically done after the fetal heartbeat is detected, which can be as early as 6 weeks gestation.

  4. MSD for estimation of gestational age: Mean sac diameter (MSD) is a sonographic measurement of the gestational sac which is usually first seen at around 3 weeks after conception (5 weeks after the last menstrual period), it initially measures 2-3 mm in diameter and grows at a rate of 1 mm a day.

  5. Evaluation of nuchal fold thickness: Nuchal fold thickness is typically measured at the end of the first trimester, around 11-14 weeks gestation, to screen for Down syndrome and other chromosomal abnormalities.

However, the following cannot be demonstrated or performed during a 1st trimester OB sonogram:

  1. LUS and cervix to check for placenta previa: Placenta previa is usually not diagnosed until the second trimester. The lower uterine segment (LUS) and cervix are not fully developed in the first trimester, making it difficult to accurately diagnose placenta previa.

  2. Sonographic appearance of the mature placenta: The placenta is not fully developed until the second trimester, so its mature appearance cannot be seen on a sonogram in the first trimester.
