A 46-year-old man was involved in an automobile accident and sustained a tear of the capsule of the carpometacarpal joint of his right thumb. In view of his history of lung disease, it was decided to repair the laceration under a brachial plexus nerve block.  WHY?The orthopedic surgeon decided to inject the anesthetic into the brachial plexus below the clavicle. DISCUSS THE STEP-WISE PROCESS FOR THIS METHOD

The brachial plexus nerve block is a type of regional anesthesia used in upper limb surgeries. It is often preferred in patients with lung diseases as it avoids the potential complications of general anesthesia, such as respiratory depression. Here are the steps for this procedure:

1. Patient Positioning: The patient is positioned supine with the head turned away from the side to be blocked. The arm is abducted at a 90-degree angle.

2. Sterilization: The skin over the injection site is cleaned with an antiseptic solution to prevent infection.

3. Local Anesthesia: A small amount of local anesthetic is injected at the skin puncture site for patient comfort.

4. Needle Insertion: The needle is inserted just below the midpoint of the clavicle, aiming towards the suprasternal notch.

5. Nerve Stimulation: A nerve stimulator may be used to confirm correct needle placement. This is done by observing for motor responses in the arm or hand.

6. Anesthetic Injection: Once the correct placement is confirmed, the anesthetic is slowly injected. The patient's vital signs are closely monitored during this time.

7. Observation: After the injection, the patient is observed for any signs of complications or adverse reactions to the anesthetic.

8. Surgery: Once the area is numb, the surgeon can proceed with the repair of the carpometacarpal joint.
